Passionate about working with high-complex medically frail clients? Ready to take your nursing career to a different level? As a?Complex Care Lead (RN) – Home Health, you will focus on:?

• Providing clinical practice support and guidance to a group of Complex Care Clinicians.
• Providing input and assist with aspects of program development, workflows, standards.
• Assisting with team care coordination and oversight.
• Facilitating team-based care, work with medical partners and other clinicians to ensure quality of care for complex clients (such as seamless transitions, holistic follow up, etc.)

In addtion to the above, you will:

• Apply specialized theory, clinical expertise and knowledge relating to designated/specific client populations, working across multiple sites/clinics/services (if applicable).
• Provide leadership, guidance and clinical expertise to the Preventative Complex Care team, a dedicated group of healthcare professionals focused on delivering exceptional care to complex medically frail older adults in their homes and throughout the care continuum.
  • Leadership is centered on fostering a team-based approach to care, emphasizing the importance of knowledge-sharing within the multidisciplinary team.
• Ensure that every team member is well-informed about the needs of each client by promoting effective communication and collaboration.?
• Serve as a valuable clinical resource for nurses and other healthcare providers, offering support and guidance to enhance their professional development.
• Lead and support the interdisciplinary team, nurture a culture of excellence, teamwork, and continuous learning, contributing to the overall growth and success of the team.
• Build strong relationships with external partners, including community organizations, to facilitate seamless care coordination for complex medically frail older adults.
• Promote effective partnerships, contribute to a comprehensive healthcare experience that addresses the unique and evolving needs of these individuals.
